Forms , Checkboxes Not Appearing Correctly

Salient Theme Uses the Select2 JS Library to Enable Fancy Select/Checkbox/Radio Styling . If for some reason the Option is conflicting with Third Party Plugins . You can turn it off in Salient Theme Options Panel - > Form Styling Tab :
