When a user adds product to the card, they can checkout; however, as soon as they start entering billing info (address, city, state and zip), the payment fields get removed. I am using WooCommerce's Square plugin to process payments. I spoke with them first. I swapped out the theme for something else and it works just fine. Can you take a look and tell me what I need to do to fix. I am using version 8.0.15
Unfortunately, that did not resolve the issue. I'll leave the default structure there for you to view on the site. Regardless of the form styles, when a user enters the city, state and zip, the payment section reloads and hides the fields.

Try Adjusting the Form Styling Options in the Salient Theme Options Panel :
