I've created two Gravity Forms on the URL - http://webfro.gs/contact-us/. The form on the right is displaying all fields except the CAPTCHA, and the form on the left is only showing the date field, CAPTCHA field, and the Submit button. The forms are created and they are both live.
I have them embedded inside two, side-by-side ThemeNectar shortcode columns.

Also - I've done a DNS flush and cleared all my cookies/cache.
Can you help me understand what I might have done wrong?

I have resolved this. The fields were not showing properly because the view was inadvertently selected as "Admin Only".
