Hi there

I want to add in my header a search field with address autocomplete which is directly linked to a Form service, such as Jotform.com or typoeform

Reason being I want the user to start typing directly in the field the first information of the form (which is address with autocomplete options)

I have already a form prepared with Jotform 

I attach a peer website for reference (even if this is with the postcard, my version would be with complete addreess and autocomplete)
