Hello, I see templates in the Salient theme that are for contact forms, however when using them, instead of a form there is a text block that says "This is where your form will go." I don't see any documentation on how to add a form there. Can you tell me how I would put a form in that space of the template?

Thanks. I'm confused on the styling options link though. Where do I enter that html? Do I create an html object with the page builder in that spot?

I'm having some problems formatting the form using the information provided below. I'm trying to have two columns using the code provided. But one field breaks across the columns. Here is my example form.

The second row, the name field crosses the columns. Is there a way to have the label for the radio buttons next to it? Also the text for the drop down menu above the drop down menu?
