Can you make sure your plugin are not the cause of the issue. Disable all except the salient related ones and woocommerce, and see if the checkout page layout is fixed.
Disable the easy login woocommerce plugin especially. If the layout is still off then give us access to the backend - username password with admin role - we check this from that side.

yes, that was this Pluggin Login/signup pop up i have deactivated and activate again and it show correct.
it something like "Magic" with wordpress...to many misterious thing happend..
i hope it block for always and doesnt change anymore
