Two brains… too much time… simple fix (of course)
My retraced steps (thinking back to how I got the problem)
I installed WP using an auto-install tool – for the first time ever (a cPanel app called Installitron)
After the install, I updated the prefix tables (the hard way… PHPmyAdmin)
I also like to customize the media path, so I set WP to use a different uploads directory. I created a directory "media" at the site root
… broken images. !?
Get this: I could get to "Edit Image" in the Media part of WP. But the smaller square thumbs in the Media area were tiny broken-image icons.
Fix: Using the FTP client, I updated the permissions on the media directories ( domain.com/media >> 2015 >> 02 >> … etc) to the standard 755 (for directories)
(I found they had 750 –– which must have been what my FTP client gave them by default instead of 755)
The fix came after checking table prefixes (several times)… and Googleing for a couple hours. :-/
After updating the theme all the image gallery thumbnails disappeared as shown on the attached file.

I also tried deleting all the images and adding the same again but same error

What ended up being the issue?
In my situation: I'm able to upload media, and it looks like the full images is there in the Media section… but all image thumbs are 'broken'
