I have Salient Theme and i would like to have a contact form dedicated to the orders like the one in the attached file.

Could you please tell me how can i make this order form in my website?

I know it's not a question about salient theme but you gave me a lot of instructions and hope you can help me even this time :) Do i need a specific plugin or do i need to expand the plugin of the ContactForm7 that i already have?

I have just a little question now: i would like to have more elements on one line (now i have one element per line).

How can i put more elements on one line in the contact form?
